Risk Warning & Liability Waiver

All non-PANSW affiliated members who attend our sessions are expected to complete our Risk Warning and Liability Waiver (below) before playing. It is however best practice for all participants to complete this at least once.

Important points:

• Casual participants must complete this form each time they play and are only able to play a maximum of five (5) games as a casual before having to join PANSW to continue to play with ATP Pickleball Club.

• Casual participants are expected to show the session host this completed form prior to participating in any of our Pickleball sessions. 

• ATP Pickleball Club membership enables you significantly cheaper session costs so we recommend you join our club ASAP!
